At the Ford dealership where I used to work, we had a technician who was not known for his thoroughness. We had a customer bring her Taurus in for an oil change and a few other minor things. He drained the oil, put the plug back in, replaced the old filter with a new one and put the car out in the lot for pick up. Later on, the customer gets it, drives around the block and comes right back in, saying it's making a weird noise. Yup, it was making a weird noise all right. The tech had forgotten to put any oil back in!! Needless to say, the engine was shot and we had to put in a brand new one for her. We didn't fire the mechanic, but he did have to pay back the 4 grand it cost us!!
